Jiangxi Provincial People's Hospital Successfully Rescues a 19-Year-Old Girl with Multiple Organ Rupture

On September 2, 2025, a 19-year-old female patient was urgently transferred to our hospital after colliding with a truck while riding an electric bicycle, resulting in severe abdominal trauma. The girl had been experiencing intense upper abdominal pain for three hours, and preliminary examinations indicated "splenic rupture," placing her in critical condition.

Upon admission, the patient presented with pallor, weak pulse, and continuously dropping blood pressure—typical signs of hemorrhagic shock. The hospital immediately activated the green channel for severe trauma management, and a multidisciplinary team responded swiftly. Departments including the Emergency Department, Trauma Emergency Center, and Imaging Department collaborated seamlessly to complete an enhanced CT scan without delay. The results were alarming: the scan revealed multiple rib fractures, gastric rupture, suspected laceration of the initial segment of the jejunum, splenic rupture, hepatic rupture, and pancreatic laceration—indicating severe multi-organ abdominal injury. With an Injury Severity Score (ISS) of 35, her condition was both complex and critical, posing an imminent risk of death due to massive hemorrhage or multiple organ failure.

Faced with such a dire situation, the team led by Dr. Fu Ping, Chief Physician of the General Surgery Group in the Trauma Emergency Center, quickly assessed the patient and made a decisive call: immediate surgery!

At 1:00 AM, the operating room lights were switched on. Intraoperative exploration revealed an even more severe condition than anticipated:

·A massive gastric rupture measuring approximately 15cm × 3cm, with multiple serosal layer tears;

·A liver laceration about 3.5cm long and 2cm deep;

·Severe splenic rupture extending to the splenic hilum, with profuse bleeding;

·A clear laceration in the initial segment of the jejunum;

·Complete transection of the pancreas at the neck, with fragmented tissue and severely disrupted anatomy.

This was a rare case of severe compound multi-organ abdominal injury, particularly involving complete pancreatic transection combined with multiple organ ruptures—an extremely perilous clinical scenario with a perioperative mortality rate exceeding 40%. Any misstep in management could lead to fatal complications such as postoperative hemorrhage, pancreatic fistula, infection, or multiple organ failure.

Confronted with these challenges, the Trauma Emergency Center’s general surgery team remained composed and executed precise maneuvers. Leveraging extensive clinical experience and exceptional surgical skill, they methodically performed a series of complex repair procedures. After five hours, the surgery was successfully completed. Intraoperative bleeding was effectively controlled, and the patient’s vital signs gradually stabilized. Currently, the patient’s postoperative condition is stable, and she was discharged on October 2.
